Letter Regarding Delta Dental

Dear Valued Patient,
Because we value you as a patient, we want to communicate about a change we are
considering that may affect you.
We are analyzing whether to continue participating with Delta Dental Insurance as a
preferred provider.
We are proud of our high standards. Continued involvement with this plan may not allow
us to continue to provide you with the excellent care you deserve for your long-term dental
health and wellness.
Please be assured, you will always be a part of our practice no matter what decision we
make regarding the plan in the coming months.
We are diligently researching the implications of this potential change and welcome your
input and questions. In the coming months feel free to contact our office as we would be
glad to discuss this with you.
If we do choose to leave the Delta Dental network, your plan could still allow you to see us
as an out-of-network provider and we are happy to submit claims for you. If you are unsure
if you can see an out-of-network provider, we recommend you reach out to your employer
or Delta Dental to see if your plan allows you to go out-of-network.
